In an essence, a hikikomori lives in a world of their own, cut-off from the world. What makes it an issue is the extent and length of isolation that they go through.

Hikikomori shuts themselves from direct social interaction. It starts with them dropping out from school or from work, and snowballs from that point onwards. They'd sever their relationships- families or friends, none of it matters. Some would even go as far as reversing their night and day to avoid socialising.

It is very common for hikikomori to lead this solitary lifestyle for months, even years.
They live but are practically invisible to society.

And this leads to an unfavourable view of them from the society.

HIKIKOMORI - A Case of the Generation's Invisible Youths explores the inner psychology and struggles of the people behind the social phenomenon in hopes of creating an understanding for them.
